Churches is also disqualified from driving until 2028 and will need to complete an extended retest.
The motorcyclist suffered serious injuries and is still recovering from them.
Thames Valley Police Roads Policing shared a dashcam video of the crash, stating: "Please, don't drink and drive."
It shows the bonnet of the car as it careers into the biker.
The biker is thrown onto the bonnet and slides off into the road, where he rolls lying on the floor.
The bike rolls into the car in front, skids and then lies horizontal in the road.
When he hits the motorcyclist, a groan can be heard inside the vehicle and then the words 'oh my f****** god'.
